Summary

We are looking for an Adult's Social Worker to join a Hospital Discharge Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast paced team who work to ensure patients are safely discharged from hospital and have much support as they need. Carrying out bedside assessments, arranging appropriate care packages for comprehensive discharges and advocating for service users and their independence is at the forefront of the team's motivation. The team work with professionals in the hospital, community resources teams and the voluntary sector.

About you

The successful candidate will have worked in a hospital team setting in the past 2 years. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of one year of experience working in an Adult's Social Work environment is essential in order to be considered for this position.
