Summary

ONGOING ASSESSMENT AND INTERVENTION TEAM - 1 x 37 hr

1-year fixed term post to cover maternity leave. Applicants must be 1-year post ASYE.



You will provide ongoing Social Work assessment, support planning and interventions for Adults aged 18+ with a range of complex needs in the Darlington area. We are based at The Town Hall in Darlington.



The team works proactively with service users, their families, carers and professionals to maximise independence through a comprehensive strengths-based approach to assessment and support planning. The dynamic nature of the team is underpinned by a strong commitment to the principles underpinning The Care Act 2014 and other relevant legislation.



The successful applicant will be required to contribute to the operational delivery of an effective Adult Social Care Service and be responsible for a range of assessments including Care and Support Needs, Mental Capacity Act, Safeguarding, and Support Planning to promote the wellbeing of adults in line with the Care Act 2014.

As a Social Worker, you will form collaborative relationships with key partners including acute, primary and community health care, voluntary and independent sector to promote health and well-being and preventing, reducing and delaying need in line with the Care Act. Through strength-based assessments and interventions and a focus on the whole person and person-centred care, you will support adults with care and support needs and their carers to achieve their desired outcomes and well-being.andnbsp;
