Summary
Specialist Diabetes Nurse or Diabetes Practice Nurse who will work within the nursing team providing care to our patient population They will be expected to: Run Diabetes clinics, reviewing bloods, medication management, lifestyle promotion, referrals to diabetes services Work alongside the diabetes practice nurse Have access to PCN specialist diabetes consultant clinic
