Crynodeb
We are looking for a nurse who has experience in reviewing patients in both an in-patient and out-patient settings, including reviewing patients in nurse-led clinics. This includes providing advice, performing investigations and monitoring treatment, assessing for oxygen therapy, providing information regarding community support and pulmonary rehabilitation. The post holder will: Attend local Multidisciplinary team (MDT) meetings, ensuring any nursing actions are acted upon and shared with the wider nursing team. To co-ordinate timely transition for patients from initial assessment to treatment. To provide information and support to ILD patients and their families at all stages of disease. To liaise and refer to the wider MDT. To ensure ILD patient data is captured. To provide support and teaching to ward and community staff caring for ILD patients. To participate in organising and running of the local patient support group. To work with the existing team in developing the ILD service.
