Summary
We are looking for a skilled professional to join KCHFT as our new Head of Service in our Transfer of Care Service.
As the Head of Service for the Transfer of care Service (TOC), you will work as part of the dynamic strategic leadership team within the East Locality Adults team and be accountable for embedding sound processes, leadership and development for the Transfer of Care Service across east Kent.
You will have overall accountability for shaping and delivering the strategic direction envisaged for the TOC and will lead quality Improvements in process design and delivery across the Locality, working in partnership with both Acute and Community partners, the Local Authority and the voluntary services .
You will lead design initiatives and engage key partners in projects that support the TOC to deliver better patient outcomes. Leadership and operational delivery of the TOC strategy will enhance the experience, safety and care for all patients. You will be responsible for ensuring that each service development supports the delivery of key objectives that align to KCHFT’s strategic direction. You will manage the LCRM for Out of Hospital Beds and be a key support for the pathway 3 strategy.
