Summary
Assess and triage patients, including same day triage, and provide treatment Run minor illness clinics, for a range of acute and chronic conditions Act as a lead for minor illness management, working closely with our Nurse Partner Undertake home visits where relevant Assist with the management of disease registers Undertake the collection of pathological specimens Maintain accurate and contemporaneous health records Take an active part in QOF management Prescribe, issue, and review medications as appropriate Collaborate with doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care
