Summary


The CHCP Wellbeing Primary Care Liaison Service are offering a fantastic opportunity for a candidate to join their team on a 12 month temporary/secondment basis as a Senior Primary Care Liaison Nurse to cover maternity leave. We are looking for exceptionally motivated, dynamic and enthusiastic individuals with appropriate experience to join our team within City Health Care Partnership Community Interest Company (CHCP CIC) Wellbeing Primary Care Liaison Service. The successful applicant must be flexible, a good team worker and used to working under pressure. They will be required to have excellent interpersonal skills and be able to effectively communicate with patients.


Main duties of the role include:
• Proving nursing and clinical leadership to the wellbeing team, including deputising for the Lead Nurse when required and undertaking the line management and clinical supervision of staff as required.
• Acting as a source of specialist information/advice regarding annual health checks for people with learning disabilities and serious mental Illness, support for accessing primary care services and implementing reasonable adjustments where patients are facing barriers to accessing health care.
• Completing clinical audit work on GP registers such as the learning disability register and serious mental illness register, making sure they are cleansed, accurate and any patients missing from registers are identified and added.
