Skills & Experience required:

Educated to A level minimum / equivalent training / experience of SEN knowledge and experience, namely Code of Practice and Children and Families Act 14.
Good knowledge of Surrey’s SEND policy and practice, national guidance and legislation.
Good knowledge of educational settings and a willingness to develop this across the 0-25 age range.
Demonstrable experience of working in a collaborative way with children/young people and parents to ensure families views are heard and applied.
Significant experience of working with vulnerable children/young people, especially those with SEND
Excellent team working skills
Ability to take ownership for writing and maintaining the EHC, and to present to partnership resource allocation forums as and when required.



What's involved with this role: As a SEND Case Officer (PRE & PFA), you will develop, review, and maintain EHC plans for children and young people with SEND, with a focus on both Preparation for Adulthood (PFA) and Preparing for Education (PRE). Your role will involve early transition planning for key areas such as education, employment, independent living, and community participation.

You will work closely with families, schools, and key agencies to create person-centred support plans, ensuring coordinated services across education, health, and care. Your work will focus on high-quality planning, assessments, and reviews, emphasizing PFA and PRE transition planning throughout each individual's journey.

Key duties:

Identify risks and opportunities related to PRE and PFA transitions, escalating to management and managing risks to support successful transitions into adulthood.
Contribute to service monitoring and review, ensuring PRE and PFA outcomes are integrated into service improvements for young people transitioning to adulthood.
Provide advice and recommendations within policy to support decision-making, focusing on PRE and PFA plans for education, employment, independent living, and community involvement.
Manage complex cases and care planning with a focus on PRE and PFA transition support, leading development and improvement projects to enhance transition services.
Build strong relationships with internal teams and partner agencies, ensuring effective coordination of PRE and PFA services and collaborative decision-making with service users, families, and carers.
