Summary

The role entails but is not limited to: Being an expert in the clinical management of patients with complex palliative care needs To support the Community team in service delivery whilst supporting the team with any clinical escalations. To act as an expert clinical practitioner, initiating advanced skills and clinical decision making. Support the medical team both across the community team and the inpatient unit Demonstrate advanced clinical skills and knowledge that informs practice decisions in a variety of clinical settings To independently prescribe medications / treatments, order investigations and independently admit to the appropriate in patient setting or refer on to another speciality To oversee and supervise others in developing their competence to confident decision making To recognise the limits of own clinical knowledge and skills and to seek medical support whenever needed to provide safe, effective, and timely patient care. To be professionally and legally responsible and accountable for all aspects of own clinical and professional activities. Provides professional and clinical leadership, providing a highly visible and authoritative presence ensuring the delivery of excellence in core The post holder will provide a corporate function in advising executive leadership team with professional nursing advice on palliative care services and service-related issues. Be part of the clinical on call rota To strive for effective partnership working with other health and social care providers in the area which supports seamless care for all patients within St Giles Hospice. To support the Director of clinical services in the delivery of the Clinical Strategy To support Quality Improvement across the organisation by leading in/participating in clinical audit/research To identify key deliverables for St Giles Hospice and the improvements required to achieve these To provide learning opportunities for nurses and other health care professionals by working alongside them, and/or via mentorship and supervision. This includes providing inter professional support for staff of all disciplines To influence strategic developments in pursuit of advancement of quality of patient care and the patient experience Contributes to the strategic and operational planning of the service and the developments of professional practice both in the organisation and across its partners involved in the delivery of health and social care locally and nationally. Support Clinical Governance across the organisation Contribute to Medicines Management working group, policies and procedures Always deliver high quality care to patients and families Recognise areas for Quality Improvement, audit and research and in turn develop plans and pathways to deliver on these Building links with other Nurse Consultants to innovate through the sharing of ideas To be current and expert in all areas of palliative care and share knowledge with all others within St Giles Hospice Support Clinical Nurse Specialists and other appropriate professionals in audit programmes to further build on the model of Quality Improvement Make a significant contribution to the clinical governance agenda by ensuring practice is evidence based and through the development of staff competence at all levels. Be key in ensuring that all care carried out by St Giles Hospice is evidence based, safe and audited regularly and appropriately Be a key member of the hospice wide clinical audit group, ensuring that quality improvement is always paramount Play an active part in Infection control, Health & safety and Equality & diversity issues. Work alongside Medical Staff in leading clinical research programmes and involve other professionals where appropriate Represent the Hospice at external networks and report on progress to Director of Clinical services Just so you know: We may review applications before the application review date, however, if you apply after the application review date, your application may not be considered.
