Summary
The post-holder will deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. Responsibilities will reflect requirements of the NHS GP Contracts, and as such may be subject to change as services adapt. A brief summary includes the following: In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and dealing with queries/tasks, paperwork, reports, referrals and correspondence in a timely manner. All clinical / admin work must always be carried out within the PA's own competence and advice sought where needed by the supervising GP. Carry out home visits on patients (where appropriate) who are unable to attend the Practice, due to their current clinical condition; Ensuring all visits are promptly actioned safely on a daily basis Undertake clinical assessment, diagnosis and treatment of patients. Analyse and interpret history, presenting symptoms, physical findings and diagnostic information, to develop appropriate differential diagnosis. Respond appropriately to patients, relatives and carers in relation to diagnosis and treatment options. Assume responsibility and accountability for own decisions and Practices and strive to attain the highest standards of Practice. Assessing the health care needs of patients with differentiated and undifferentiated / undiagnosed problems seeking help from a clinician where needed. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Screening patients for disease risk factors and early signs of illness. In consultation with patients and in line with current practice disease management protocols, developing care plans for health within in own competence. Providing counselling and health education within own competence. Referring patients to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. Collecting data for audit purposes when requested Requesting in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate and if qualified to do so.(All prescriptions are reviewed by a prescribing clinician before being issued) In general, the post-holder will be expected to undertake a large range of normal duties and responsibilities associated with working within primary care with supervision and support where needed.
