To provide specialist IBD advice and support to multi-professional teams to improve the way in which services are provided. To assess IBD patients specialist health and well-being needs at all stages of their illness. In liaison with the wider multidisciplinary team plan, deliver and evaluate treatment to address patients complex and changing health needs at all stages of their illness To assess when the patient may need to switch medications using Therapeutic Drug Monitoring Identifying patients and adding them to the appropriate MDT to discuss ongoing symptoms, imaging results or complex and changing symptoms that may require surgical intervention. To be responsible for managing a case load safely, asking for advice where appropriate and maintain comprehensive, accurate patient records. To assess patients needs in relation their specific IBD needs and to plan, implement and evaluate care delivery. To develop a wide understanding of IBD and the available medical/ surgical treatments to ensure a high level of education can be offered to the patient and their relatives. Co-ordinate and provide clear and comprehensive planned care and information to patients and the multidisciplinary team. Provide specific dietary and health education advice for patients. Provide advice on lifestyle issues e.g. work, sports, travel, relationships. Provide ongoing support to patients through the telephone help-line, the IBD email service, ward visits, the bleep and nurse-led clinics. Attend and actively contribute and lead various meetings relating to the IBD Nursing service; to enhance good communication and liaison between team members to ensure a smooth pathway of care for the patient and take forward agenda actions as required. Attend Consultant-led Gastroenterology and surgical clinics to provide joint consultation in order to contribute to patient care programmes. To request relevant and appropriate endoscopic and blood test investigations; to include radiological and MRI investigations following completion of IRMER training and on recommendation of the medical team. Liaise with and advise medical colleagues regarding patient care, to include advice regarding timely admission and discharge of patients. Maintain accurate patient records of telephone calls by documenting on Cerner and maintaining comprehensive records to ensure continuity of care. Ensure confidentiality at all times. Comply with Data Protection and Trust policies. Deliver patient care with patients consent and co-operation, being sensitive to their need for courtesy, dignity and privacy. Be accountable for own professional actions whilst working autonomously in the role. Maintain networking with colleagues outside the organisation to develop knowledge and keep up to date with IBD nursing issues. Act as advocate for IBD patients within the hospital and the wider community Ensure that all equipment used in your practice is in good working condition and report any defects or malfunctions appropriately Ensure stock items under your control are ordered and available for use. Be aware of the cost of equipment and clinical consumables and take responsibility for safe and prudent use.
