Summary


An exciting opportunity has arisen for a knowledgeable, motivated Discharge Keyworker to join Durlston Ward at Poole Hospital.

This post offers the successful individual the opportunity to enhance their discharge knowledge and practically support both patients and staff ensuring that patients experience a safe discharge.
The successful individual will spend the majority of their working day with the MDT and liaising with patients, relatives and the wider discharge network to streamline complex discharges:

You will be expected to:

• Support the ward MDT with assessment and decision-making.
• Liaise between the ward staff, therapists, social workers and external partner staff.
• Guide ward staff to improve discharge planning referrals/ process.
• Support effective discharge planning reporting via the discharge related IT systems.

• Complete appropriate referrals and discharge paperwork

• Book joint therapy and family meetings.

This role is part time, 22.5 hours per week, set days: Monday, Wednesday & Friday.

Base Location: Poole Hospital
