Summary
CARDIO-PULMONARY EXERCISE TESTS (CPET) Undertake pre-test assessment to identify any contraindication, potential risks, or special precautions prior to exercising. Ensure the resuscitation trolley is adequately stocked and date checked. Perform and interpret full 12 lead CPET, exercise oximetry and 6-minute walk tests. Accompany the patient throughout exercise (this may range from brisk walking to cycling) and continually monitor the patients condition using full clinical data (V02, AT, VC02 etc), 12 lead ECG, oxygen levels, psychophysiology, blood pressure and visual assessment of patient in order to determine whether to carry on or terminate the test. BLOOD SAMPLING Perform blood gas measurements (capillary/arterial) according to national guidelines BRONCHIAL CHALLENGE USING MANNITOL/METHAOCHOLINE Undertake pre-test assessment to identify any contraindication, potential risks or special precautions to be taken, ensure that the patient has no response to the Mannitol/Methacholine. Assist with undertaking, interpreting, and reporting on bronchial hyper responsiveness Monitor the patients response ensuring their results have returned to baseline levels and administer nebulised bronchodilator if required. SLEEP & VENTILATION SERVICE Set up, issue, download, analyse and interpret multi-channel sleep studies, this includes giving the patient detailed instructions on how to set up the monitor at home. Set up and issue and download NIV/CPAP treatment to diagnosed OSA patients, including mask-fitting service. Advise the patient and relatives of the nature of the lifelong treatment and the requirement for a successful trial, encourage full compliance for the trial period. Perform a specialist practitioner role, assesses and treat own workload of patients with sleep disordered breathing using NIV/CPAP therapy, maintaining associated records. Issue permanent machine following post trial, explain the results to the patient, write report for the notes and organise a follow up appointment Undertake physiology led follow up outpatient clinics for NIV/CPAP. Check compliance and reduction in symptoms. Take clinical history and alter treatment as required. Advise medical staff of change required and further investigation as indicated. Motivate non-compliant patients using knowledge of disease process to explain the importance of the treatment and provide advice, support and practical solutions for the problems preventing good usage. Clinical Duties Produce accurate and comprehensive technical reports on the range of tests performed and ongoing interpretation of the results of the procedures with a view to rebooking for further investigations
