Summary
Support Farrow Medical Centre medicines optimisation strategies. Identify medicine related problems or queries including supply and availability and work to resolve any such issues. Undertake medication reviews and consultations to the scope of ability and practice, and with support, mentoring and coaching, expand to a wider scope. Liaise with other healthcare professionals to assist in the continuity of excellent patient care across primary and secondary care. Perform all duties in a highly professional and efficient manner including answering the telephone and face to face contact. Undertake all tasks in an accurate and timely manner.
