A Vacancy at University Hospitals of Leicester NHS Trust.

This is a post in Paediatric Intensive Care and Transport Medicine. Children’s Medical Emergency Transport (CoMET) service is hosted by UHL & operates from a central transport hub at Castle Donington alongside the neonatal, adult and ECMO transport teams. The successful applicant will be based at Castle Donington & will have opportunities to rotate to the General and Cardiac Paediatric Intensive Care at Leicester Royal Infirmary.

The post is suitable for trainees who are looking to enhance their intensive care and Transport skills in a specialist intensive care/ECMO unit; either as a stand-alone fellowship, an out of programme arrangement or following completion of their CCT. Applicants will need to have completed core training (ST4 and above), and ideally have experience in neonatal and/or paediatric intensive care at registrar level. Previous experience in critical care transport, in the post-operative care of paediatric cardiac patients & experience in ECMO would be an advantage. Applicants from a paediatric, paediatric emergency medicine or anaesthetic/Adult ICM background would be welcome.

The new CoMET base has a dedicated simulation suite with state-of-the-art immersive audio-visual facilities and a simulated ambulance on order. This advanced simulation facility is used to deliver both in-house and external training on various elements of transport medicine. Successful candidates will have the opportunity to participate in and further develop this education program. They will also be part of outreach education provided in district general hospitals.

Support the day-to-day running of the clinical activities in the CoMET Paediatric critical care transport service. Once signed off, you will be a part of specialist team transporting critically ill patients by road and by air. You will liaise with referring hospitals regarding retrieval and repatriation of patients. To attend/present at regular transport meetings including governance, morbidity and mortality meetings, clinical practice meetings and case reviews.

Support the day-to-day management of patients in the general and cardiac Paediatric Intensive Care Unit. You will become a key member of a highly skilled multidisciplinary team. You will work under the direction of the Paediatric Intensivist. Alternate weeks will be based on Comet and the intensive care unit. A dedicated unit placement can also be arranged. Participate in the weekly pre-operative cardiac surgery forums, monthly clinical practice and clinical risk meetings, morbidity meetings, and other activities relevant to the running of a high-quality critical care service. Participate in audit activities.

It is expected that trainees will sign up to the RCPCH ePortfolio or equivalent where they have not already done so.

Detailed job description and main responsibilities:
You will be part of our established transport team and will gain experience in transport of critically ill children by road and by air. You will liaise with referring hospitals regarding retrieval and repatriation of patients. Gain expertise in transport of patients needing intensive care support by providing mobile advanced respiratory care and hemodynamic support. Various learning opportunities during CoMET placement include- transport ventilation both invasive and NIV(HFNC and CPAP/BiPAP), palliative care transport, procedural skills (airway’s/lines), Point of care Ultrasound skills, enhance leadership skills leading a small specialised team.

To provide safe and effective care to children and families that require urgent or planned transfer for uplift in care or repatriation to the local hospital. To work effectively and safely as part of a regional transport service. Ensure good and clear communication with referring and receiving clinicians.

To work as part of the paediatric transport service and paediatric Intensive care service ensuring appropriate systems are in place to evidence ongoing compliance with local and national policies as required for CQC standard. Trainees will be required to complete the transport log book to demonstrate fulfilment of transport competencies.
