Summary
Lead and have operational responsibility for the clinical management of the Kingfisher outreach Service Facilitate a learning environment that supports others to develop advanced clinical skills Provide an enhanced role and expertise which aims to prevent unnecessary acute hospital admissions and (where safe and appropriate) keeps patients in their own homes, facilitating effective hospital discharge from The Kingfisher inpatient service Demonstrate enhanced competence in complex decision making, assessment and the management of clinical needs Proactively monitor standards and lead innovative processes to improve quality and the operational management of the unit/service Be an accountable and responsible point of contact for clinical quality Ensure the team works in accordance with organisational, regional and statutory guidelines, as well as best practice guidance as outlined in formal benchmarks or standards Occasionally the post holder may be required to support staff on The Kingfisher. They will also be required to act as an autonomous senior clinician being accountable for a defined caseload Clinical Practice Leadership Be a clinical Leader across the community environment, responsible for the operational management and accountable for the quality of all clinical activity within the designated area, Hold a small caseload of highly complex service users for time limited interventions, assessment and triage Complete/support person-centered assessment Ensure all service users have a SMART, person-centred, evidence-based and holistic care and treatment plan supporting service users to reach the optimum level of independence and that robust processes are in place to keep such plans under continual review Ensure service users are able to take an active role in the assessment, management and evaluation of their care and treatment plan, as well as all relevant risk and personal safety plans Empower service users to be at the centre of their care Lead the team to establish, embed and maintain a sustainable programme of psychosocially-informed therapeutic engagement Lead the team to offer individualized care and treatment interventions Set, embed, audit and maintain high standards for clinical record keeping Address concerns, supporting people to resolve problems and overcome barriers effectively, Communication & Relationships Work closely with key internal and external personnel including, social care, housing, voluntary and non-statutory agencies. Many of the service users will present with long-term and/or deteriorating conditions, which might result in distressing and emotional situations. The post holder will be required to develop teams skills and support team when these demanding situations occur, utilising de-brief processes and other specialist support as required. Finance, Workforce and People Management Objective Manage all relevant clinical, pay and non pay budgets Take responsibility for clinically-relevant aspects of the physical environment Contribute to the review of staffing resource and skill mix, ensuring that service provision is both safe and therapeutic Develop staff rosters in line with AWP guidance, delegating the clinical and administrative elements of this appropriately Contribute to the planning and development of clinical services, including the review of team establishments, grade mix and skill mix, and to be actively involved in all aspects of recruitment Monitor and manage performance and conduct in accordance with Trust policy Participate in the investigation and administration of relevant HR and recruitment processes in the Locality e.g. grievance and disciplinary hearings, redeployment processes, recruitment events, team-transfers, rotations and retention initiatives etc, Other Deputise for the Matron when required Promote a positive and open culture which supports equality and values diversity and inclusion Undertake manual handling and physical behavioural management adhering to best practice principles and utilising specialist training The ability to work in a flexible manner, both proactively and reactively to situations that arise on a daily basis is essential
