Summary

Main Duties and Responsibilities To provide a nurse facilitated service for patients within the Directorate using care protocols. To provide and initiate appropriate levels of care within agreed guidelines and protocols to enhance the care of patients. To order and interpret investigations using the agreed Scope of Professional Practice, local policy and national Guidelines. To identify the clinical abnormalities that will impact on the patients journey and in conjunction with the Lead Practitioner develop new protocols and guidelines. To contribute to the development and implementation of initiatives ensuring a high standards of patient care with current best practice guidelines. To provide support and information to patients and their relatives on all aspects of their care prior to admission and on discharge, including Health Education through acute episodes of care. Document patient discharge needs and communicate information to receiving area. To collate and maintain accurate records including investigations, results, outstanding investigations and current plan of care. To ensure that the Nursing and Midwifery Council Code of Professional Conduct is maintained by other professional members, acting and reporting any deviation as per Trust policy. To act within the Trusts standard competencies and guidelines using extended skills with Scope of Professional Practice guidelines and Competencies. To work closely with the on- call T&O team seeing patients in the ED facilitating timely admission or discharge Research To underpin and highlight new practices and implement project plans for future development of the service. To assist in the auditing of current practices and clinical care through the Essence of Care clinical benchmarking. To identify and participate in the appropriate research to improve and develop the service. To contribute to the development of emergency care pathways and service development. Education and development To be responsible for personal professional development, ensuring up to date knowledge base, and to keep documented evidence of maintained and continued development. To underpin new practices with the relevant education and information. To be responsible for the development and implementation of educational programs for all staff ensuring that high standards of care and life-long learning are adhered to. To act as an effective role model and source of support, demonstrating effective leadership skills in order to promote a high-quality motivated workforce according to Trust Core Values. To act as a mentor/ supervisor when appropriate utilising inter-professional working relationships.
