Summary
To deliver high quality care to patients with complex needs within the community setting, working with patients to establish personalised care planning goals, encourage self-management (particularly those with Long term conditions) and support independent living. The complex care nurse will work with patients, their carers and families to develop a personalised plan of care and will be responsible for planning, proactively monitoring and anticipating the changing needs of these individuals, co-ordinating their care across all parts of the health and social care system. The complex care nurse will hold overall responsibility for a designated caseload within their allocated locality. The postholder will work with a multidisciplinary team to include the patient's registered general practice team, district nurses, community hospital teams, secondary care, social care, mental health and the voluntary sector. If relevant you may also be involved in annual reviews for care home residents. This post will be hosted by Somerset NHS Foundation Trust as the employer. Service delivery will be managed by the West Somerset Primary Care Network team and the postholder will report to the Neighbourhood Service Lead. Please see attached JD for full details.
