Skills & Experience:

Prior experience or knowledge of substance use services would be an advantage
2+ years working within a substance misuse type role.
Transferable skills such as: managing and prioritising workload, delivering interventions, care planning and risk management.
Excellent communication and interpersonal skills.



What's involved with this role: We are looking for a Family Caseworker to join our Family and Young Persons Team to coordinate and support families affected by substance use by delivering tailored interventions such as advice, harm reduction, and psychosocial support. The role involves working with key stakeholders like social care and schools to ensure holistic, effective support packages, covering hidden harm interventions including 1-1s, group, family, or remotely.

Key responsibilities:

Conduct initial screenings and assessments to determine clients' needs and eligibility for services.
Gather comprehensive information about clients’ substance use history, mental health, and social circumstances.
Identify appropriate interventions, services, and packages of support tailored to individual client needs.
Collaborate with team members to develop and implement recovery plans based on assessment findings.
Carry out risk assessments and implement risk management strategies.
Perform proactive outreach, including home visits and community appointments, to enhance engagement and promote social inclusion.
Identify and address barriers to treatment, developing innovative solutions to enhance accessibility.
Ensure clients have regular and up-to-date copies of their support and safety plans.
Maintain accurate records of clients' assessments, progress, and plans.
