Summary

1.1 Main Responsibilities and Duties To show dynamism, enthusiasm and flexibility in participating in the reshaping of acute medical services at Epsom and St Helier University Hospitals NHS Trust and to work closely with the Clinical Lead and Clinical Director of Medicine. 1. To help provide a first class clinical service to all Trust patients. 2. To provide high quality leadership and support to medical staff, allied health care professionals, and Trust management and support staff in both the Acute Medicine Service, and associated medical specialty areas. 3. To support and sustain teaching and research where possible in relevant clinical areas. 4. To work within the framework of applicable Trust policies, procedures, and clinical governance framework. 5. To work within contractual requirements, and in support of applicable Trust clinical development frameworks, business plans, and development strategies. 6. To consistently treat all patients and colleagues with respect and professionalism and to adhere to the principles of the trust regarding equality and diversity. 7. To share in the organisation and provision of high quality clinical care for acute medical patients within the Trust. 8. To assess and review patients within the setting of the Acute Medical Unit and SDEC, including performing appropriate clinical procedures in this setting. 9. To facilitate the early discharge of acute medical patients, including where necessary, by early review in the Acute Medical Unit/SDEC review clinics, where clinically appropriate. 10. To provide senior support and input into clinical decisions taken by junior medical staff within the Acute Medical Unit / SDEC. 11. To ensure the provision of accurate and timely communication between the Acute Medical Unit 12. / SDEC, other units within the Trust (including critical care), and in primary or intermediate care settings. 13. To work to provide smooth, timely and appropriate transfer of patients to appropriate speciality beds within the Trust. 14. To participate in the development and implementation of care pathways for common medical emergency presentations. 15. To participate in the out of hours medical on-call rota at weekends (the intensity is site dependent), such on-call including assuming clinical responsibility for the care of all medical patients within the Trust (including those outside of the Acute Medical Unit). 16. If requested and agreed with the Clinical Directors for Medicine, to contribute to a sub-specialty service in the form of clinics, procedural lists and specialist ward referrals. 1.2 Principle Duties of the Post The principal duty of this post is to provide a comprehensive clinical service to patients. 1. To at all times, comply with the GMCs guidance on Good Medical Practice as amended or substituted from time to time. 2. To ensure that patient confidentiality is maintained at all times. 3. To undertake duties as defined within the attached job plan in so far as practicable and in agreement with your Clinical Director. 4. To be responsible and managerially accountable for the reasonable and effective use of Trust resources that you use and influence, and professionally accountable to the Medical Director. 5. On commencement to ensure personal attendance at the Trusts corporate Induction (or arranged Consultant Induction). 6. To participate in the Trusts Statutory & Mandatory training programme. 7. To be responsible for the clinical management of in and out patients under your care. 1.3 Teaching & Training 1. To share responsibility with other Consultants for the recruitment, appointment and supervision of junior medical staff in accordance with Trust policies. 2. To devote time on a regular basis for teaching and training of junior medical staff, contributing as appropriate to post-graduate and continuing medical education. 3. To devote time, through agreement on a regular basis for teaching and training of medical students as part of agreed teaching arrangements with the Foundation School. 1.4 Management & Governance To be responsible for the management, appraisal and discipline of junior doctors who are attached to you, after taking appropriate advice, where appropriate and to be involved in future service development. 1.5 Job Plan: A provisional representative timetable has been included below. This is subject to change with regards to days to meet the needs of the departments but the dedicated PAs to each specialty will be consistent. Formal week plan will be negotiated following appointment. The job plan will be a prospective agreement that sets out a typical working pattern, duties, objectives and responsibilities for the coming year. It will cover all aspects of professional practice including clinical work, teaching, research, education, managerial responsibilities and external commitments. On appointment of the post holder, the Clinical Director will commence discussions to finalise the job plan which will become operational on appointment. The job plan is an indicative plan of the duties the post holder is likely to be asked to undertake. Additional programmed activities may be offered to the post holder. Any offer of additional programmed activities will be based on the needs of the service and in line with the Trust objectives and will be reviewed on an annual basis. 1.6 Private Practice The post holder should ensure that any private practice commitments are discussed with your Clinical Director and entered into the prospective job plan. Any change to these will need to be discussed between the post holder and the Clinical Director and recorded in an amended job plan. Clinicians will be required to ensure that they always meet the code of conduct for private practice. 1.7 On Call Arrangements The post holder will participate in the on call GIM rota on a 1 in 12 basis. The post holder will provide on call for medicine out of hours. At the weekends this will include post take and mid take ward rounds. The on call rota is currently defined as Category A. Consideration of time off will also be approved for weekend working when appropriate. Additional working hours over and above that of the job description will be permitted time off in lieu as appropriate. Category A: this applies where the clinician is typically required to return immediately to site when called or has to undertake interventions with a similar level of complexity to those that would normally be carried out on site, such as telemedicine or complex telephone consultations; Category B: this applies where the clinician can typically respond by giving telephone advice and by returning to work later, except in an unforeseen emergency 6.9 (currently St Helier site only with plan to extend to Epsom site in the future) AMU weekend Review (9:00 13:00) (1:9) Attend 9:00 am for MDT meeting in AMU. Targeted ward reviews of sick patients, medical reviews or potential discharged that have been highlighted by medical team/nursing staff. You will be supported by medical SHO. 6.10 Administrative Arrangements The successful applicant will have secretarial support and office accommodation.
