Summary
Integrated Neighbourhood Teams: Establish and support the implementation of Integrated Neighbourhood Teams (INTs) that align with place-based priorities and the Fuller Stocktake. Concept of Place: Champion the concept of place across the ICS, ensuring local strategies reflect the shared vision for integrated care delivery. Practice and PCN Visits: In collaboration with the General Practice & Integrated Care Steward, link with all practices and PCNs within the locality, supporting alignment with INT development, share best practice and identifying improvement opportunities. Health Inequalities: Lead the delivery of targeted interventions addressing variations in access and outcomes for underserved populations. Workforce Development: Mentor clinical leaders and collaborate on workforce transformation initiatives. Patient Engagement: Lead community engagement events annually, incorporating feedback into local plans Local and System Priorities: Support the delivery on a local level of agreed system or ICB priorities.
