Summary
You will be an excellent communicator, maintain good working relationships with the team, patients, carers and service users ensuring key information is communicated in a timely, robust and efficient way. You will work closely with ward staff, Trust teams and liaise with relevant agencies to coordinate the administrative procedures associated with discharge. You will liaise with patients and carers and support risk identification.
