Summary
As a Care Assessor, you’ll be the first point of contact for new clients, ensuring their needs are fully understood and that they receive the right support. You’ll play a vital role in shaping personalised care plans and ensuring every client experiences the highest standard of care.
Your Responsibilities Will Include:
✅ Conducting assessments for new clients to determine their needs and preferences
✅ Developing and updating person-centred care plans
✅ Liaising with families, healthcare professionals, and other key stakeholders
✅ Ensuring all assessments and care plans comply with regulations and best practices
✅ Supporting a smooth transition for new clients into the day centre
What We’re Looking For
✔ Experience in a care assessment, care coordination, or similar role
✔ Strong knowledge of person-centred care planning
✔ Excellent communication and interpersonal skills
✔ A compassionate and professional approach
✔ A relevant Health & Social Care qualification (Level 3 or above preferred)
