This is a full-time role, working 37 hours per week, on a Maternity Cover basis.
Day to day duties of the role
Proactively delivery the Specialist Support Model of inclusive practice and implement the SEND Code of Practice.
Planning and reviewing individual programmes of support ensuring all aspects of provision and support specified within a learner’s ILP and/or Education Health and Care Plan is in place and the progress is effectively tracked and monitored.
Carrying out high quality Initial Needs Assessments and diagnostic assessments along with other appropriate assessments to establish support needs and effectively track progress leading to successful outcomes.
Arranging and deliver transition programmes to ensure seamless transitions between FE and HE.
Required skills and qualifications
LDD Specialism e.g. foundation degree in LDD or other specialist qualification.
Working Knowledge of Additional Support Funding.
Relevant and Successful experience in learning support.
Skills in the development and implementation of Individual learning programmes.
