Summary
The post holder will ensure that the review of the health care needs of individuals referred for continuing health care (CHC) and Section 117 aftercare (S117) eligibility. This is met per the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care (FNC). The post holder will collate and undertake comprehensive and co-ordinated assessments and decision making for NHS Continuing Healthcare, NHS-funded nursing care and S117 with clients, carers and colleagues within health and social care. The post holder will also care plan and commission services to meet the needs of eligible clients. The post holder will ensure timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost effective.
