Summary
Day to day responsibilities will include: - Reviewing all inpatient bed requests clinically in order to insure the request is appropriate. - Line manage and work alongside the bed coordinators to ensure the daily bed management caseload is up to date with all relevant clinical updates. - As part of the MDT the post holder will identify service users care needs, making recommendations regarding these care needs and assess clinical risk as required. - The post holder will need to be able to bring together stakeholders, with differing viewpoints and priorities, to reach consensus on individual discharge plans. - Lead the co-ordination of service user care in complex cases, admissions or transfer as appropriate, ensuring both health and social care needs are fully assessed. - Recognizing when problems may occur and initiating an escalation to the service manager in a timely manner. - To have an understanding of all clinical TOCH roles in order to deputies for each other. - Participating in relevant health promotion aspects of the service, including facilitating Inter Agency Meetings and maintaining the Inter Agency Network. - Contribute to the development of policies and procedures within the TOC Hub. - Maintain adequate records as required by existing procedures, entering appropriate details on Service Users case notes as necessary, show a willingness to attend training to facilitate recording information electronically.
