Applicants are welcome for an Advanced Physiotherapy Practitioner (APP) role within the Rheumatology Service and Musculoskeletal Physiotherapy team at Chelsea & Westminster Hospital NHS Foundation Trust.

Advanced Physiotherapy Practitioners (APPs) use highly specialist knowledge and skills to provide healthcare autonomously to patients.

This MSK/Rheumatology APP role will independently clinical assess, diagnosis and treat patients with undiagnosed and undifferentiated conditions. In addition the APP is also responsible for appropriately referring patients to other relevant specialties, requesting any necessary investigations as well as facilitating safe patient discharge to primary care.

The role benefits from multidisciplinary support and supervision, including supervision from a nominated Rheumatology consultant and peer support from both an APP in a similar rheumatology role at our West Middlesex Hospital site and our team of MSK APPs.

Key responsibilities
• Independently assess, order and interpret investigations, diagnose and implement a treatment plan for patients presenting to Rheumatology and MSK services.
• Review patients for response to treatments, effectiveness of medication within scope of practice and liaison with the medical team.
• To be professionally responsible for all aspects of practitioners own work including clinical, educational, leadership and research.
• Assist in the leadership and management of MSK Physiotherapy
service where required, including supervision of senior MSK physiotherapy staff, MSK pathway design, quality improvement and staff education.
• To maintain excellent communication with the Rheumatology MDT, MSK Physiotherapy Service, GPs, operational managers, patients and other relevant healthcare professionals.

Roles and responsibilities
Clinical
• Uses highly developed clinical knowledge to independently assess, plan,
implement and evaluate treatments and interventions for patients presenting to the Rheumatology clinic with Axial Spondyloarthritis.
• Undertakes physical examination as indicated by the patient’s condition; autonomously analysing complex clinical signs.

•Request investigations eg blood tests, ultrasound scans and guided injections, X-Rays and MRI scans in accordance with IR(ME)R regulations and local non-medical policies.
• Analyses multiple sources of data including patient history, physical
examination and investigation findings when making diagnoses, clinical
judgements and evaluating care provided; presenting this information to
specialist /senior medical staff for advice if scope of practice is reached
• Continuously revaluates findings and clinical response to treatment and
establishes an appropriate ongoing management plan accordingly.
• During patient consultation discusses medication for therapeutic effectiveness appropriate to patient need within the role’s scope of practice and in liaison with the medical team and prescribing pharmacist.
• Assesses patients with mental health needs using local policy and guidelines and refers to the appropriate services.
• Communicates effectively with patients, carers, colleagues and others using appropriate communication styles. Anticipates barriers to communication and ensures patients and significant others are kept fully informed and consent to treatment.
• Acts as a resource for staff, advising on local, national and Trust policy,
procedures and guidelines ensuring patient safety and clinical governance.
• Be professionally and legally responsible for all aspects of your work, including the management and prioritisation of clinical caseload working as an autonomous advanced physiotherapy practitioner to ensure provision of an efficient and effective Rheumatology service.
• To provide comprehensive written records of assessment and treatment in medical notes and physiotherapy notes according to trust standards,
including providing a high standard report/clinic letter as indicated.
• To act as a source of expertise and point of contact for the MSK physiotherapy team in the management and treatment of patients with Rheumatological diagnoses and maintain a clinical physiotherapy caseload.
Professional
• To comply with the physiotherapy codes of professional conduct at all times
• To be professionally and legally responsible and accountable for all aspects of the practitioner’s professional activities.
• To be responsible, with the divisional therapy lead, for leadership in
maintaining and improving all aspects of clinical governance, evidence-based practice and quality assurance in MSK/Rheumatology practice
• To be responsible for maintenance of accurate and evaluative written records, to include comprehensive and timely progress reports, clinic letters and referrals.
• To be responsible for own personal professional development and keep
abreast of new advances in musculoskeletal and Rheumatology clinical practice and also support and oversee physiotherapy staff learning.
• To participate in the appraisal system as both appraisee and appraiser to more junior staff using trust documentation.
• To contribute to clinical teaching and staff development of MSK peers, medical students and other health professionals within the Rheumatology team.
• To effectively manage complaints and conflict scenarios, both within specialist clinics and MSK physiotherapy department.
• To understand and comply with all mandatory training requirements and ensure your team are compliant with training.
Organisational
• To be responsible for providing an efficient and effective physiotherapy service, by demonstrating good leadership, organisational, time management and prioritisation skills.
• To teach, supervise and appraise therapy staff within the MSK Physiotherapy team and assist the therapy lead with any complaints / formal HR processes.
• To manage and prioritise own workload by appropriately balancing clinical and other demands to ensure all service needs are met.
• To formulate flexible strategies for clinical education and service development within area of physiotherapy expertise.
• To assist with management of the MSK physiotherapy service, alongside MSK colleagues including patient care, training, supervision, various meetings eg management and strategy planning and meetings with other health professionals.
• To use excellent prioritisation and time management skills to meet the
unpredictable and conflicting needs of the service.
• To work closely with consultants, GP’s, service managers and other healthprofessionals to provide a seamless service.
• To maintain accurate and up-to-date patient records in line with departmental, professional and legal standards, and ensure that all members of your team do likewise.
• To be aware of and implement all organisational, departmental and Trust policies and to be involved in updating any as appropriate.
• To ensure timely communication of assessment and treatment findings to other involved disciplines such as discharge and referral letters and ensure that all members of your team do likewise.
• To assist in the operational management of MSK physiotherapy.

Communication
• Communicates with enthusiasm and conviction; motivating, inspiring and encouraging.
• Utilises and demonstrates sensitive communication styles to ensure patients are fully informed and consent to treatment.
• Communicates effectively with patients and carers; recognising the
need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

• Communicates effectively; often about complex, sensitive and potentially distressing information with patients, family and/or carers.
• Creates a trusting partnership with patients and/or relatives/carers in order to communicate and explain complex medical issues (including new diagnoses) and agree a management /treatment plan.
• Ensures awareness of sources of support and guidance such as PALS and provide information in an acceptable format to all patients recognising and referring any difficulties and referring where appropriate.
• Communicates clearly and effectively with colleagues; often receiving or giving complicated plans or information of a sensitive nature.
• Maintains the responsibility for the delivery of a detailed, accurate and
potentially complex clinical handover to colleagues in other specialities and primary care e.g. GPs. Produces detailed and accurate written
information within the medical notes regarding all clinical assessments,
investigations completed and requested, and treatments administered
• Maintains confidentiality as required by professional, local and national policy. Acknowledges situations where there can be a breach of confidentiality.
• Recognises and defuses potentially aggressive and violent individuals/situations in line with local policy and legal frameworks.
Education & Research
• Takes responsibility for their own learning and performance including theparticipation in clinical supervision and maintaining an awareness of relevant research evidence.
• Develops health promotion and education in conjunction with other health care professionals ensuring that all patient care within the multi-disciplinary team is based on research and best practice.
• Acts as a constant source of clinical and theoretical knowledge for all grades and disciplines of staff as well as patients and their significant others, providing support and clinical advice in specialist areas, based on evidenced based research.
• Possesses proficient typing, IT and computer skills. Uses audio/visual
equipment to record and impart information and research.
• Prepares and delivers presentations and participates in workshops on a local / regional / national level as required to ensure the communication of good practice.
• Works with the MDT to further develop appropriate clinical pathways and care approaches. Disseminates their own learning and good practice to other team members.
• Designs, coordinates and undertakes collaborative audit, research and
development into the specialty and related practices.
• Plans, delivers and implements programmes of education and training for trainee APPs, nurses, medical and other disciplines and continues to be involved in their ongoing support.
• Supports the Trust’s research and development plans
