Summary

To deliver asset-based solutions as part of the wider ‘Make Every Adult Matter’ (MEAM) team. Hold a small case load of individuals, liaising with other professional services, identifying barriers that prevent fair and equal access to services.

Job Responsibilities

1. To work in partnership and collaboration with Changing Futures MEAM partners.
2. To navigate across multiple services support for individuals experiencing multiple disadvantage across the borough of Rochdale
3. To provide asset based and solution focused interventions to relieve disadvantage
4. To appropriately challenge the system when it is not working for people and assist in finding viable solutions.
5. To build strong, supportive relationships with a small case load of beneficiaries
6. To build strong professional relationships within the Rochdale MEAM network partners, stakeholders, and partners.
7. To capture and report interventions provided on the data system GM Think, including case studies
8. To report on and capture the impact of the support provided and work done
9. To participate in awareness raising and other campaigns which promotes the work we do to promote system change and the MEAM approach.
10. To provide asset based and solution focused interventions to relieve disadvantage
11. To provide written reports and presentations on the work as appropriate.
12. To take part in Changing Future team meetings and other Changing Futures/ MEAM network meeting as appropriate.
13. Maintain and update skills and knowledge in housing and benefits sectors and awareness of changes in relevant legislation.
14. To keep up to date records of work undertaken in the delivery of the service.
15. To take part in team meetings and other management meetings as and when requested.
16. Engage in personal supervision, appraisals, training and CPD as required
17. Work at all times within Sanctuary Trust policies and procedures
18. Undertake relevant tasks outside normal responsibilities as directed by senior management
