Our Crisis Resolution and Home Treatment teams are multi-disciplinary and provide a community based mental health service. CRHTT aim to provide a safe and effective home-based assessment and treatment service as an alternative to in-patient admission. We can facilitate early discharge from the ward which provides an opportunity for patients to receive care and support in their own homes which is the least restrictive environment. The team’s interface with other statutory and voluntary services in order to achieve the best outcomes for patients during HTT episode of care. The team work on a shared caseload basis which means our team members do not carry a specific caseload themselves, instead they share one as a collective unit.



The teams are resourced to provide care and brief crisis intervention 7 days a week for people who are experiencing a mental health crisis. The Service support adults from the age of 18 which is recovery focused and work in collaboration with clients whilst supporting carers according to individual need. The service is available to adults from the age of 18 who present in acute mental health crisis.

Staff will undertake rapid assessments of client’s mental health and social care needs, they have access to a array of resources in the community and will either refer or signpost to other services. For client’s presenting out of hours, there is a crisis Mental Health support Line who will provide support from the hours of 10.30 to 8am.

Merton and Wandsworth Crisis Resolution and Home Treatment Teams are keen to recruit registered mental health professionals who are interested in working as part of a dynamic multi disciplinary team.

We are currently looking for full time Nurses or mental health practitioners registered with HPCP or Social Work England. We are particularly keen to hear from B6 mental health practitioners who aremotivated and committed to be part of an exceptional service and be a proactive member of a multi disciplinary team. The successful candidates will have the opportunity to use evidence based crisis intervention skills to support clients in mental health crisis and work collaboratively with clients and their carers to access on going support in the community if required. The successful candidates will be based at either Springfield Hospital in Tooting or The Wilson Hospital in Mitcham and will be expected t work a combination of early and late shifts including weekends. In addition to the benefits of working as part of a multi disciplinary team, the right candidates will be offered regular supervision and a confident autonomous practitioner. You must have good communication skills, be organised with an ability to prioritise competing tasks. Ideally a car driver/owner is essential for navigating around the respective geographical areas.

Key responsibilities:-

- Assessing clients health and social care needs who present in acute crisis.

- co-ordinating effective gate keeping assessments when needed.

- providing a range of bio/psycho/social interventions for service users and carers

- planning for a robust, sustainable discharge as well as sufficient aftercare

- Completing collaborative care and crisis care plans with clients and their carers.

- Leading shifts, involving fair delegation of work to colleagues.

- Maintaining professional registration and keeping abreast of relevant mental health policy and evidence based mental health practice.
