Summary
To maintain independence and daily living skills of inpatients by providing high quality therapeutic groups and individual activities under the direction of the Occupational Therapist
To provide information to the ward team to enable ongoing assessment and contribute to safe effective discharge plans. Activities may be undertaken in community settings as directed.
To provide support to patients to enable them to maintain independence in personal, domestic and social skills.
To provide person centred support taking into account physical, psychological needs, personal preferences and choices of individual patients and their carers.
To provide an activity program for the ward that will meet the needs of patients who suffer from functional illnesses, this to be done in liaison with occupational therapist and physiotherapist.
To be involved in the assessment of inpatients and carry out a programme of specified group activities as directed by the nurse in charge in liaison with physiotherapy and Occupational Therapy staff.
To contribute to specified individual assessment activities as directed by the nurse in charge including personal care and domestic assessments.
To work closely with all Great Oaks team members to ensure that a high standard of communication is maintained regarding service user needs and progress.
Attend clinical and non-clinical meetings as required by the service.
