Key duties of the role include: CARE HOME LINK Act as the primary point of contact for designated care homes, building effective working relationships with care home staff, residents, and families. Conduct comprehensive nursing assessments for care home residents, including long-term condition reviews, medication reviews, and frailty assessments. Coordinate multidisciplinary team (MDT) reviews, ensuring all stakeholders are involved in care planning. Support early identification and management of deterioration in residents health Provide advice on evidence-based clinical interventions, including end-of-life care and palliative care management. CLINICAL NURSE DUTIES Undertake clinical duties such as wound care, immunisations, chronic disease management, and phlebotomy where required. Deliver proactive care, including health promotion and preventative healthcare for residents. Liase with and refer to the appropriate services for ongoing patient care management.
