We have a fabulous opportunity for you to join our extra care Community Wellbeing scheme in Leeds. We support older people with care and support needs to live independently in their own homes. We have 64 apartments with a variety of on-site facilities making it a real hub within the local community. We are minutes from the A1M and very close to excellent public transport links - super handy!
Typical day as a Lead Reablement Worker
- As our Lead Care Worker (known internally as Lead Reablement Worker), you’ll support our customers at Greenmill Gardens. They have a range of support needs including frailty and dementia.
- You’ll help customers maintain their independence. You’ll work with them to develop their care plans as well as coordinating and guiding our team of Reablement Workers to deliver on these plans.
- From leading on caseloads, risk assessments, support planning and working collaboratively with external agencies, no two days are the same!
- You might help with personal care in the morning and support customers cooking their lunch in the afternoon. Some of our customers will have more complex needs such as stoma and catheter care.
- In the event of an emergency, you will be the first point of contact for our customers. Customers will rely on you to respond swiftly and sensitively to any emergency or unexpected support needs.
You bring
- You’ll be passionate about making a big impact on our customers’ lives. Your empathy will shine through as will your ability to empower customers to live their best life.
- It’s important that you have worked in a supported housing environment before. If you’ve previous experience in an older person, extra care or reablement service that would be a bonus!
- You will be willing to provide personal care and have an excellent understanding of safety, risk management, CQC requirements and legislation in relation to personal care.
- You’ll have a great understanding of professional boundaries, safeguarding, case management and creating personalised support plans.
- To help us keep up-to-date records and our customer safe we need you to be technologically savvy (or at the very least not a technophobe!) so that you can use our systems.
- You’ll hold a minimum Level 2 Diploma in Care. If you hold a Level 3 that would be awesome but if you don’t, we can support you to achieve this!
Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external candidates for this role. This remains under regular review.

Job details
- As we provide 24/7 support to our customers, you’ll work on a rota basis with shifts being either 7am – 3pm or 2:30pm – 10:30pm.
- For this role you’ll work Wednesday, Friday, Saturday and Sunday.
- We need you to be flexible to meet the needs of our customers but will find the shifts that suit you and give you the time you need to spend time doing the things that matter to you!
- You’ll work closely with our housing management team to develop wellbeing activities for the customers that accommodate their needs.
- You’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we’ll pay for this.
