Summary


The Nursing Associate will:

• Be responsible for delivering high quality, patient centred, compassionate care under the direction of a Registered nurse/senior Outreach case manager with a focus on promoting health and independence

• To engage people with substance misuse problems to enter treatment services, providing therapeutic interventions across the area in group and 121 settings.

• Be an accountable professional

• Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual

• Carry out specific clinical and care tasks and responsibilities to a high standard and competency,

• Have the ability to work without direct supervision, at times delivering care independently in line with the individual’s defined plan of care, within the parameters of practice of the Nursing Associate role, accessing clinical and care advice when needed.



The responsibility of the postholder is to participate in the planning and delivery of individual care provided by the multi-disciplinary team.

They will work independently under the supervision of the Registered Nurse/Senior Outreach case manager, providing holistic evidenced based high quality care to patients and will be professionally accountable for their actions.

They will supervise and delegate appropriately to support staff within the team

• Maintain a professional portfolio, identifying role and skill

developments in line with the NMC Code of professional

practice.

• Keep up to date with current national developments in nursing practice and policy

• Ensure NMC revalidation requirements are met

• To contribute to Trust wide/departmental policies, procedures and guidelines.

• Maintain a professional appearance in line with the Trust dress code

• Uphold the Trust’s values and behaviours and behave in a manner fitting with the responsible position of the post holder, maintaining public confidence and the Trusts reputation for delivering excellent care

• Maintain professional working relationships with staff and respect the equality and diversity of each and every person he/she comes into contact within the course of his/her business.

• To assist in the decision making process regarding the referral of service user’s children to children’s services assessing and interpreting complex and conflicting information

• Deliver high quality, compassionate care under the direction of a Registered Nurse (or other registered care professional dependent on setting) with a focus on promoting health and independence
