Summary

As a trained professional you will be working alongside a wider family support team to provide crucial support to both patients and family members coping with one of life's most difficult moments. You will be joining an experienced and innovative Multi-Disciplinary Team and will help to identify and meet the emotional, practical, social and financial needs of those we support both patients and their families. You will be working alongside an experienced Social Work Assistant.The post will involve assessments and making referrals to external agencies and providers to ensure we provide the right support, at the right time and considering the appropriate legislations. Please note this post is full time working on-site at the Hospice. Please note the successful candidate will be required to undertake an enhanced level DBS check.
