Summary

Care Co-ordinators work closely with GPs and other Primary Care Healthcare Professionals within the Practice to provide a patient centred approach in identifying and managing a caseload of patients, ensuring that appropriate support is made available to them and their carers, and making sure that their changing needs are addressed. They focus on delivery of local health priorities, health inequalities or population health management risk stratification. The Care Coordinator will be part of the Practices Multi-Disciplinary Team (MDT) and will be responsible for the co-ordination of and providing support to the monthly meetings, identifying and prioritising patients for review. They will be responsible for managing the care of patients registered with the Practice. This will involve coordinating the work of healthcare professionals and non-clinical staff, including volunteers involved in the care of patients registered at our Practice. They will play a key part in co-ordinating services in care homes and the wider community with an aim to provide and improve continuity of care for patients and their families adopting a holistic approach and by acting as a point of contact for residents, families and professionals who visit care homes, such as MDT members and in-reach specialists. The post holder will contribute to tackling inequalities in health and social care particularly regarding individuals with Long-term conditions including over 65, Learning Disabilities (LD), Severe Mental Illness (SMI) and Dementia as well as cancer patients. An ethos of promoting independence and partnership-working is integral to this post. The post holder will require a degree of flexibility as the range of duties may vary as our services develop. IT/Record Keeping Keep accurate records of LTC, LD and SMI patients and their current care needs, including call and recalls for any chronic disease clinics, Prostate-Specific Antigen tests (PSA), DMARDs Carcinoembryonic antigen test (CEA), Testosterone monitoring and vaccinations. To record accurately appropriate SNOMED codes and any other codes that are used for reporting purposes to ensure good practice and maximum income generation. To provide reports as and when requested. On-going maintenance of multiple patient Registers. Support the Practice Manager and Lead Nurse with regards to the Nurse/ PCN clinical rotas. To liaise with IT to develop good systems of working to improve continuity of care for example with incoming letters/tasks etc. Develop and maintain patient waiting lists as and when required. Communication and working relationships Demonstrates ability to work as a member of a team. Is able to recognise personal limitations and refer to more appropriate colleague(s)when necessary. Actively works to build good relationships and create a network of contacts that can assist in the job role within and outside the Practice. Works with people at all levels and disciplines effectively, including service users, Primary Care Network Practices and partners e.g. Care Homes to ensure new referrals are logged and allocated accordingly. Develop excellent working relationships with the partners, wider service networks including the voluntary sector, GP practices, adult social care, hospitals, community pharmacists and other members of the MDT. Provide background information about individuals for the MDT meetings. Maintains and communicates effectively with families/carers, residents of care homes and professionals who visit the care homes, and other staff both internal and external and members of the public. Actively signposting team members, service users and carers to relevant services. Effectively recognises and manages challenging behaviours presented by carers or relatives. Create patient newsletters to provide service information and updates to patients using own initiative with regards to content. Assist in reviewing and maintaining the practice website, and internal communication system, to enable effective communication with regards to healthcare, awareness, vaccination, screening campaigns and anything that is required contractually in line with the ICB England. Oversee and assist in ensuring that the information displayed in the Health Centre is done so in a clear, coherent and compliant manner, providing useful and helpful assistance and information for all patients. Quality To ensure that work is carried out within the clearly defined protocols, policies and procedures. Recognises scope of job role and does not act beyond this boundary. To assist in streamlining processes and producing efficiencies by creating, updating and maintaining standard operating procedures (SOPs) in own areas. Provide accurate information in a timely manner to patients and their carers/relatives. Personal/Professional Development The post-holder will participate in any training programme implemented by Swallownest Health Centre or Rother Valley South Primary Care Network as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. Attend any associated training courses relevant to the role. Undertake training for any qualifications as required by the role. Take responsibility for own development, learning and performance and demonstrate skills and activities to others who are undertaking similar work. To share best practice with colleagues across the PCN. Health and Safety/Risk Management The post-holder must comply at all times with the organisation and Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System. The post-holder will comply with the Data Protection Act (1984), The General Data Protection Regulations (2018) and the Access to Health Records Act (1990). The post-holder will comply with all necessary training requirements relevant to the role as identified by the organisation. Special Working Conditions The post-holder may be required to travel independently between other practice sites within Rother Valley South Primary Care Network, or beyond (including home visits for patients registered at network practices) and to attend meetings etc. hosted by other agencies as and when required.
