Crynodeb
Our Palliative Care Specialist Nurses work independently as part of the well-established community team at Ashgate Hospice, supporting patients and carers with palliative and end-of-life care needs across North Derbyshire and Derbyshire ICBs. The community team work closely with Ashgate multidisciplinary team and wider system to deliver patient-centred, evidence-based specialist services, ensuring high-quality care. The team consists of Palliative Care Specialist Nurses, Palliative Care Nurses and Palliative Care Support Workers and are an integral part of Specialist Palliative care services at Ashgate Hospice. The role involves providing patient-centred, evidence-based care and encompasses clinical duties, as well as leadership, management, education, audit, and research responsibilities. Flexibility is required, including availability for weekend and bank holiday shifts.
