Summary
A Care Coordinator, or Patient Care Coordinator works with both patients and Healthcare Professionals to ensure seamless, organised and effective care. This role involves working closely with patients and healthcare providers, to facilitate communication, manage appointments, and support patient care plans. Key Responsibilities: Patient Liaison: Serve as the primary point of contact for patients, addressing their queries and concerns, and ensuring they understand their care plans. Care Coordination: Coordinate patient appointments, referrals, and follow-up care, ensuring timely access to services and continuity of care. Communication: Facilitate effective communication between patients, healthcare providers, and external agencies to support integrated care delivery. Record Management: Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with data protection regulations. Support Services: Assist patients in accessing support services, such as social care, mental health services, and community resources. Linking in with our Social Prescriber Quality Improvement: Participate in quality improvement initiatives to enhance patient care and service delivery. Team Collaboration: Work collaboratively with healthcare teams to support patient-centred care and contribute to multidisciplinary meetings. Reception: Help out on reception when needed taking calls from patients and making appointments Qualifications: Education: Educated to GCSE Experience: Previous experience in a healthcare or care coordination role is preferred. Skills: Strong organisational, communication, and interpersonal skills. Ability to work independently and as part of a team. Attention to Detail: Ability to manage multiple tasks and maintain accuracy in a fast-paced environment. Problem-Solving: Strong problem-solving skills to address patient needs and coordinate care effectively. Empathy and Compassion: Demonstrated ability to provide empathetic and compassionate support to patients and their families. IT Proficiency: Proficient in using healthcare information systems and standard office software is preferred. Full support and training will be given.
