Summary
To provide specialist learning disability knowledge, expertise and input to support the work of the access to healthcare for people with learning disabilities, to ensure that people with learning disabilities living in West Sussex can effectively access primary and secondary health care services as required, resulting in improved health outcomes. To support implementation of the Directed Enhanced Service (DES) Annual Health checks for people with Learning Disabilities, through provision of specialist education and support to Primary Care Staff to ensure that practices are able to meet its requirements and to promote uptake of the DES amongst practices across West Sussex. To identify training needs for local services and people with learning disabilities and their carers and deliver as appropriate in conjunction with people with learning disabilities.
