Support the Consultant in Rehabilitation Medicine and act as liaison with his multi-disciplinary clinical team

Be responsible for nursing assessment, triage and providing nursing care to patients, this will include pre and post major limb amputation and associated disorders

Responsible for medicines management, wound care, AED, anaphylactic provision and infection control

Demonstrate an advanced ability to apply complex skills and knowledge commensurate to a higher level of professional practice

Provide specialist advise and guidance, education and support to patients, carers, medical and nursing staff, Allied Health Care Professionals (AHPs) and voluntary agencies

Facilitate multi professional and inter sectoral collaboration, working across acute, community and voluntary sectors

Responsible for planning, delivering and sign posting to other agencies, evaluating the personalised health and wellbeing care needs of patients, in line with the Trust’s Values & Nursing & Midwifery Council Professional Framework goals example: personal hygiene and comfort, early recognition and management of acutely ill patients, safe transportation, wound dressings, removal of sutures/clips, compression hosiery, psychological and spiritual support

Evaluates patients, assisting in the Prosthetic Service clinics

Actively involved in supporting Therapy Support Assistant and Students

Balancing all aspects of the job role, being flexible and responsive to competing demands

Ensure the effective and smooth delivery of the nursing service within the Services for all patients with lower and upper limb amputation and/or deficiency



Promote a safe environment for patients within the clinical settings



Be responsible for reporting effectively (both written and verbal) on the care and condition of patients and where appropriate, initiate any action that may be required, maintaining client confidentiality at all times, e.g. safeguarding



Be responsible for the triage of all services referrals, planned preventative maintenance reviews and those seeking advice on stump management



Be integral to the assessment of primary patients attending the Services, agreeing Care Plans with the patients and keeping contemporaneous records, both physical and electronic (ELMs)



Offer patient advice on a range of matters including smoking cessation, management of substances use/misuse, healthy living – signposting to other support services as necessary



Communicate via telephone and face-to-face with patients and carers, on a daily basis, some of which display severe challenging behaviour and to assist with personal care

Actively maintain relationships and liaise with colleagues, hospital staff, GP’s, community nurses, AHP’s, care managers, Social Services and any other external agency, to ensure the provision of an effective nursing service is maintained



To take an active role in all clinical audit activities as a member of the Clinical Team, including the Inter Regional Audit Group (IRPAG)



Participate in delivering Services organised training sessions for external AHP’s and onsite training delivery from associated organisations and products Webinar sessions



Support health care professionals and students visiting the Services on placement



Be conversant with Nursing and Midwifery Council Advisory documents: `The Code of Professional Conduct, Administration of Drugs and the Guidelines for Professional Conduct



Undertake role of Lead Link for Infection Control and Medicines Management



Responsible for monitoring diabetic clients and liaising with external agencies



Undertake wound assessment, liaising with external agencies to enable effective wound management



Ensure clinical equipment is maintained, liaising with the Lead Specialist Equipment Co-Ordinator for Medical Devices Management, EME, etc.



Manage FP10’s, dressings, drugs and associated supplies, liaising with the Business Administration Hub for orders to be placed



Undertake, and maintain compliance with, all mandatory training and seek opportunities to support own continual professional development (CPD), maintaining knowledge of new developments and concepts, particularly related to Tissue Viability



Be a member of the Nurse Amputee Network group (35 members)



Adhere to Trust policies and procedures in the delivery of care and contribute to ongoing review and implementation of new policies, practices and procedures by keeping professionally updated
