Summary
The Stroke Nurse Specialist will contribute to the delivery of a high quality service for adults who have had a stroke in the Dartford & Gravesham NHS Trust area. The post holder will be required to provide patient-centred clinical care, encompassing the skills of assessment, examination, investigation, diagnosis and treatment, within an agreed scope of practice, throughout the Acute Stroke/TIA/Thrombolysis Service, leading to the safe referral and discharge of patients across the hospital and community. As senior nurse within the stroke service, you will be integral in forming the model of delivery for the Service that will achieve a cohesive, effective and seamless pathway for the stroke patient journey. A key element of this role will involve the education, training, advising and support for a variety of staff and stakeholders involved in delivering stroke care within the Dartford & Gravesham NHS Trust area.
