Summary


This is a London Borough of Havering position -full time 36 hours per week.
To supervise the Support, Time and Recovery (STR) and Peer support workers (PSW) Team.
To focus on the direct needs of service users.
To work across boundaries of care.
To provide support, give time and thus promote recovery to clients with more complex needs.

We are looking for a flexible and dynamic individual who enjoys working in a multi-disciplinary team and has the capability to work independently in the community. We would like someone who has experience of working in a community setting, who has worked independently with adult Mental Health clients. We arrange regular appointments with our clients to provide one to one support to develop ADL's. Our aim is focus on client recovery to enable people to live independently and achieve their desired goals.

Main duties of the job

To supervise, as directed by the Social Inclusion and Resources Manager, the Support, Time and Recovery Team and Peer support workers.
To be responsible for a small caseload of service users. To establish supportive and empowering relationships with the service user and carer.
To work in isolated situations with service users who may be vulnerable or at risk and also to support team members to do the same.
To liaise with other agencies and any other relevant professional, and to direct STR/PS Workers in doing so.
To be responsible for monitoring changes, both positive and negative within the lives of those who receive services from the STR/Peer support Team as a whole, and notify the Support Services Manager.
To take appropriate action in cases of emergency, and to offer direction to STR and Peer support worker Team in consultation with the Support Services Manager

The post holder would be expected to lead in the set up, facilitation and overseeing of our community groups to encourage social integration.
Our working hours are 9 - 5 Monday - Friday (Ad hoc weekend and bank holiday working). The base can be one of mental health and wellness teams located at either:

(South MHWT) Oasis House, 28-30 Gubbins Lane, Harold Wood Essex RM3 0QA

(North MHWT) The Petersfield Centre, Petersfield Avenue, Romford Essex RM3 9PB



Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

The primary purpose of the role is to:

· Contribute to the delivery of an effective Adult mental health MDT service as part of a social care service. Improving outcomes for People and Carers.

· Carry out high quality holistic assessments, using strengths based, person-centred, community focused, and multi-agency interventions to bring about positive change through developing effective relationships and to secure the best outcomes for people and carers.

· Understand the legal and statutory duties of the post as required by the Care Act. Have a good awareness and understanding of Mental Capacity Act, liberty protection safeguards, Mental Health Act and any other relevant legislation.

· Provide a person-centred service which focuses on asset-based interventions to enable people to optimise and maximise their abilities, by using a strengths-based approach.

· To pro-actively support and work collaboratively with health services to ensure people receive the required health and social care support within the community, supporting the facilitation of are discharged from hospital within agreed timeframes with appropriate support mechanisms in place, prevent hospital admission.

· Support people to maintain their independence within their homes by utilising support in the local community and the persons personal networks.

· To be responsible for a small caseload of service users to establish supportive and empowering relationships with service users and carers.

· Support the appropriate allocation of resources required to maintain independence through the use of Personal Budgets and explore all options that are available to people.

· Contribute to maintaining quality standards, and ensure self-development is maintained.

· Manages, plans, prioritises and progress cases of an appropriate level of complexity, seeking support and suggesting resolutions for workload difficulties with due regard to service standards, targets and timescales.

· Promote effective communication within the team and with colleagues internal and external of the organisation.

· Assess and evaluate risk in partnership with service users, carers, and the wider MDT where required putting in place appropriate interventions, strategies, and contingency plans to mitigate risk and to protect vulnerable adults.

· Use knowledge and skills to support appropriate recovery focused care plans, promoting choice and independence.

· Assessing and using professional judgment to assess level of need and urgency, as well as obtaining further information as required, determining appropriate intervention and proposed outcomes of intervention.

· Promote and deliver new ways of working and evidence-based practice within the service.

· Develops and implements reflective practice in all areas of work.

· Implement evidence-based practice in all areas of practice and recording.

· Promote and implement social care principles and practices within an integrated setting with health.

· Clear understanding of Mental Capacity and impacts on practice.

· Support the team to meet performance requirements and outcomes and ensure data is recorded in line with organisational requirements.

· Ensure safeguarding issues are raised in line with the Havering’s policies and procedures.

· Ensure that vulnerable adults are adequately protected in line with Havering Safeguarding Adults policy and procedures and relevant legislation.

· Have a good understanding and working knowledge of risk; including identification and management of risk for the people they are working with and to self.

· The post holder will respond to the needs of the service by being positive, flexible, and empathetic to customers and carers’ needs, working in an integrated way supporting both social workers and occupational therapists to promote independence, choice, and control.

· Strengths based practice will be the key driver to providing excellent person-centred care whilst applying the eligibility criteria and ensuring equity across the service.

· Develops and implements reflective practice in all areas of work.

· Support and be actively engaged with the induction of new staff.

· Act as a point of guidance and reference for appropriate persons within the team.

· Ensure that staff expenditure is properly authorised and recorded, in line with departmental procedures under the guidance of the social inclusion manager.

· To instigate, participate and facilitate service development including social activity groups in the community in consultation with the social inclusion manager.

· Post holder may be required to perform duties not specifically identified in the job profile but which are in line with the general responsibilities of the post.

· To be responsible for monitoring changes, both positive and negative within the lives of those who receive services from STR and peer workers and reporting to the care co-ordinator, team manager and social inclusion manager.
