Summary
Clinical responsibilities To be responsible for providing a comprehensive evidence-based dietetic service for patients and carers. This will include: The provision of a comprehensive and effective dietetic service, providing nutritional counselling to patients on designated acute wards and community sites, in conjunction with the supervising clinical specialist. To provide expert advice to patients, carers and clinicians regarding provision of nutritional support for patients based on clinical evidence. This will involve nutritional assessment; to include the calculation of nutritional requirements based on the interpretation of biochemistry, anthropometry, clinical condition and other physical parameters. To provide nutritional counselling to patients and carers in the outpatient setting on appropriate dietary treatment and management where training in post registration cognitive behaviour therapy is required Ability to develop and use communication and teaching skills to educate the diverse and multicultural population both on a one to one and in groups. To directly prescribe nutritional supplements, vitamins and mineral supplements as agreed with the medical team. To communicate and liaise with the community multidisciplinary team regarding nutritional management of patients and where necessary providing information and education to clinicians and carers. To advise and liaise with catering staff concerning the special dietary requirements of inpatients, and to assist in instructing chefs on the preparation of therapeutic diets as required. To provide nutrition teaching sessions to other members of the multi disciplinary team on the following topics: care of the elderly, nutrition screening, coronary heart disease, COPD and all appropriate health promotion initiatives both in hospital and the local community To be responsible for the discharge planning of patients going home on artificial nutritional support. To work as team to prioritise a large caseload and support each other to deliver quality, safe and efficient patient care Professional Responsibilities To participate fully in the appraisal process and develop a strategy for continuing professional development encompassing a portfolio based assessment in line with national legislation. To be an active team member of the dietetic department at Guys and St Thomas participating in departmental developments and projects and attending staff and continuing professional development meetings. To carry out administrative duties in line with local standards and policies including ensuring adequate patient records are kept and all requirements for data collection are met. Education & Training To undertake appropriate training and then participate and supervise the clinical training of Dietetic students. To participate in the development and delivery of training to any appropriate staff group. To undertake training to be deemed competent in the use of all equipment related to nutritional assessment e.g. anthropometric equipment. To be competent in the use of information technology with particular reference to nutritional packages for assessment of patients nutritional requirements and dietary adequacy To undertake training to be deemed competent in the use of enteral feeding pumps and equipment. To give talks/lectures to interested groups Research/Audit To undertake supervised research or audit, where appropriate, which will enhance dietetic care for the patient. To undertake such other duties as shall be indicated by the Clinical Dietetic Services Manager to meet the exigencies of the service and/or needs of the patients.
