Summary

An opportunity has arisen for an Adult Social Care Worker (ASCW) in our dedicated Local Authority CHC Team.

Are you interested in developing a good working knowledge of the National Framework for Continuing Healthcare, providing support and guidance across adult social care teams, and being part of a small specialist Local Authority team supporting the CHC process? We are looking for an Adult Social Care Worker to join our small team of Social Workers supporting social care and health colleagues to deliver good outcomes for people engaged in the CHC assessment process.

About the role

We are a small but innovative team; We oversee the CHC process from a local authority perspective. We work across all the adult care teams in North Somerset and collaborate with our Health colleagues daily.

You will be working within key legislative frameworks including the Care Act 2014, the Mental Capacity Act 2005, as well as the NHS National Framework for Continuing Health Care.

As a team we provide information and guidance in respect of Continuing Health Care to adult care teams across the council. You will be required to support and advise staff to identify when it is appropriate to complete CHC screening.

You will be required to represent the local authority at the Continuing Health Care assessments.

You will be involved in multi-disciplinary meetings and decision-making processes.

You will use your knowledge, skills, and values to promote the rights and choices of people with care and support needs.

This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.

Work Style

At North Somerset Council, we offer a hybrid working environment based on four distinct work styles. Depending on the role and personal circumstances, you will fall into one of these categories. However, please note that during the induction period, it may be necessary to spend more time in the office to support your learning and development. We are flexible and open to discussions about your preferred working style, and if you wish to be in the office more frequently, this can be negotiated.

This role falls into the category of Mobile/Field worker which means that although your office base will be at the Town Hall, Weston-Super-Mare, you can expect to work in the following way:

0-1 day in the office
Includes field workers who primarily work remotely.
Comes into the office for meetings or focused work. Virtual connections with the team are essential, with portable technology supporting work on the go.

About you

You will have a minimum of 1 years’ experience of working within social care and/or supporting people with social care needs.

You will have knowledge of the legislative frameworks as outlined above and be willing to further develop an in depth working knowledge of relevant legislation.

You will have, or be willing to develop, a good understanding of The National Framework for Continuing Healthcare (revised 2022) and of NHS Continuing Health Care checklists and full CHC assessments.

Candidates would need to have, or be willing to develop, specialist knowledge in this area within an adult care setting. You will have the ability to confidently work within a multi agency arena. You will build relationships with health colleagues, to deliver effective joint working for the people we work with. Excellent communication skills are essential to the role.

Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.
