Would you like to be able to work flexibly around your other commitments, working as part of a multidisciplinary team providing holistic care for patients in their own homes and community settings?
We are seeking enthusiastic Community Nurses of all bands to join our staff bank for ad hoc work. We can offer shifts across community sites based in Leatherhead, Dorking, Banstead, Epsom and East Elmbridge including our twilight services, with hours between 08.00 – 23.20. We also have opportunities to work in our two community hospitals based in Dorking and Molesey, as well as our integrated unit at Epsom hospital covering 24/7. We can offer a supportive and flexible working environment, along with competitive rates of pay.
Community Nurses deliver care in patients homes and residential homes as part of a Multi-Disciplinary Team, linking closely with the GP Practices and other community services in the local areas. We work with the aim of preventing unnecessary admissions to hospital and in assisting people to manage their conditions in their own homes.

• Venepuncture
• end of life including medicines - injectable / syringe drivers
• Ivabs ( preferable not essential)
• wounds - compression preferable not essential
• dopplers preferable not essential
• wound dressings - ulcers & accidental wounds
• Pressure area management and knowledge
• PIC lines - flush and management
• PEG - meds and flush preferable not essential
• bowel care - Suppositories / enema / DRE
• Insulin administration
• injections
• bp/pulse /sats - identifying a unwell patient
• Catheters - wash out / inserting / removing : Male / Female/Spubic .
• Bladder Scans

· Act as an autonomous practitioner, working with the support of Senior Team Members.
· Treat all patients as individuals, respecting their privacy and dignity at all times
· Assess individual care needs, implement and evaluate holistic programmes of care
· Involving, supporting, informing and educating family/carers
· Take every opportunity to promote health and wellbeing of the patient
· Develop effective communication and liaison systems
· Maintain, develop and monitor standards of evidence based nursing care
· Be responsible and maintain timely patient care
· To work in co-operation and partnership with all other professionals and agencies involved in the care of the patient
· Be aware of and act upon when necessary, procedures that are in place to protect vulnerable individuals
· Maintain own professional and clinical integrity in line with NMC guidelines.
· Ensure continuity of care between all clinical settings through appropriate timely planning and good communication through all available channels
· Maintain clear and comprehensive, signed and contemporaneous records according to Surrey Downs Health and Care procedures.
· Ensure up to date knowledge in all aspects of clinical practice, and act as a resource and provide support and guidance for other staff.
· Be responsible for ensuring that policies and procedures and standards of care, are adhered to at all times.
· Support the team lead where appropriate and assisting with delegated tasks and duties requested to support the service needs.
· Be aware and act upon, when necessary procedures that are in place to protect vulnerable individuals. Liaising with safeguarding team and social services and support other team members in this process.
