Summary
Provide professional advice and support on the safe prescribing, handling and administrationof medicines. This includes support regarding development and implementation ofmedicines management processes and policies. Lead on the medicines aspects relating to the care of patients to optimise medicines, reducemedicines-related risks and improve adherence to taking medicines by carrying outmedication assessments and reviews in identified patients. Resolve day-to-day medicines and prescription issues with healthcare professionals Help patients to manage their conditions Lead repeat prescription services and deal with queries from staff and patients Present to Patients any changes to prescribing in line with NICE, best practice or in responseto prescribing alerts Advise patients of the adverse side effect of medication and potential interaction with othermedication/treatments Rationalise repeat prescriptions lists to avoid waste and duplication Provide reviews to those patients with LTCs or complex health needs, including drugmonitoring and risk versus benefit. Provide both face to face and digital appointments to practice patients Contribute to the management of the prescribing budget through the promotion of safe,cost effective rational prescribing Contribute to minimising the harms associated with the use of medicines, for example by deprescribing
