An exciting opportunity has arisen for an experienced heart failure nurse to be part of the heart failure/cardiology team at UCLH. We are looking for someone who is driven, enthusiastic and innovative with excellent experience in the care and management of patients with heart failure. Flexibility across the cardiology specialism will be needed to support services and provide CNS crossover with consultant support, where necessary.

Fundamental to the role is working closely with the Heart Failure Consultants and the wider cardiology and medical teams and ward staff. Regular multidisciplinary team meetings, plus working alongside consultant colleagues, will provide the post holder with ongoing access to expertise and clinical support. Clinical duties will include:

· identifying suitable HF inpatients for early supported to discharge via the UCLH Hospital at Home team during inpatient ward rounds, including liaising with colleagues and case finders

· liaison with the Hospital at Home team to deliver expert heart failure care to patients under the pathway in the community

· weekly outpatient clinics to deliver care to patients with all forms of heart failure.

These duties will be undertaken in close collaboration with cardiology consultants and advanced nurse practitioners.

This is a fixed term contract and requires a five-day week commitment (37.5hr) without night, weekend, or bank holiday working. There are no home visits involved.
