Summary

Clinical: Undertake comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action. To assess, evaluate and manage clinical risk escalating these concerns appropriately when required. To formulate a plan of care based on identified need. Deliver evidence-based interventions as appropriate. Review effectiveness of care plan at regular intervals. To work collaboratively with service users, carers, and partner agencies. To maintain computer held care records in an accurate and timely fashion. Undertake all work in a culturally sensitive manner. Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate. Develop effective working relationships with Primary Care colleagues. Undertake Team Management role in the absence of the Team Manager. Professional: Responsible for participation within the Trust appraisal process and to take a lead in the identification and engagement regarding their own personal development, professional mandatory & supervisory training needs. The post holder is responsible for ensuring that they access and participate in clinical supervision. Responsible for the safe management and administration of medicines as defined by Trust policy. To participate and contribute actively within research, service development & clinical governance. Is responsible for ensuring that confidentiality is always maintained in accordance with the data protection act, trust policy & best practice. Participate in joint working with appropriate service & agencies. Responsible for maintaining a professional manner when interacting with patients, carers, and colleagues. Supervision: Regularly review own caseload with supervisor to promote efficacy and contribute to the overall management of Service. Deliver supervision and facilitate PDR with more junior members of staff. Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution.
