An exciting opportunity has become available for newly qualified nurses and Social Workers in Ealing Southall MINT. You will be supported through either a preceptorship programme (nurses) programme. This role is perfect for nurses that are keen to learn new skills quickly and advance their careers and become a band 6 worker. You will be joining at an exciting time as we transform our services following the Community Mental Health Framework. This framework represents a new approach to situating and providing place-based integrated support, care and treatment in the community for people with mental health problems whatever their level of need.
Community mental health nurses/social workers play a key role in dissolving the barriers that can exist between mental health and physical health, social care, voluntary and community social enterprise (VCSE) and local communities You will be working in primary and secondary care, to deliver integrated, personalised, place-based and well-coordinated care for adults and older adults. We are moving towards a ‘whole system’ of mental health care, which will provide timely access to support, care and treatment.
We need Community Mental Health Nurses to support people with mental health needs to access care where and when they need it and be able to move through the system easily. This role means supporting people to be able to manage their problems and move towards individualised recovery on their own terms, surrounded by their families, carers and social networks, and supported in their local community. We need Community Mental Health Nurses who can support individuals be able to contribute to and be participants in the communities that sustain them, to whatever extent is comfortable to them.
