Summary


An opportunity has arisen for a Band 4 Health and Social Care Assessor within the Bee at Home team.

Assessment skills and medication management experience would be advantageous.

This is a full time post 37.5 hours per week compromising of 3 x 12 hour shifts and 1 x short shift.

The service hours are 8am - 10pm, 7 days a week, 365 days a year.

Applicants must be flexible to cover the service hours. Previous applicants need not apply

To work within the Domiciliary Care Service undertaking initial client assessments and screening initial referrals received via Local Authority or directly from clients enquiring about services provided. The post holder will be required to assess and determine the nature and level of care required by individuals referred to the service.

There will be a requirement to regularly review and update packages of care being provided.
