Summary
Provide compassionate and empathetic care and support to patients with life-limiting illnesses and their families, respecting their dignity, choices, and cultural preferences. Assist patients with personal care tasks, including bathing, toileting, dressing, and feeding, ensuring their physical comfort and well-being. Support patients with mobility, transfers, and positioning, using safe manual handling techniques and equipment as required. Facilitate late discharge and support overnight admissions into hospice care. Monitor patients' vital signs, symptoms, and general condition, reporting any changes or concerns to the Head of Hospice at Home. Document patient observations, care interventions, and other relevant information accurately and timely in accordance with hospice policies and procedures. Assist patients with activities of daily living, including meal preparation, serving, and feeding, ensuring their nutritional needs are met and dietary preferences are respected. Support patients in engaging in recreational activities, hobbies, and social interactions to promote their well-being and quality of life. Maintain a clean, safe, and comfortable environment for patients, ensuring their rooms and communal areas are tidy, well-stocked, and free from hazards. Provide emotional support and companionship to patients and their families, actively listening to their concerns and offering reassurance and empathy. Collaborate effectively with multidisciplinary team members, including nurses, doctors, social workers, and therapists, to ensure coordinated and holistic care for patients and families. Participate in team meetings, case conferences, and care planning sessions, contributing insights and observations to inform patient care plans and goals. Provide supervision and guidance for volunteers, working with the Volunteer Manager when necessary to maximise volunteer impact.
